ICD-10-CM vs ICD-10-PCS: which code set do you need?
The names differ by two letters and cause endless confusion, so here is the clean split:
| ICD-10-CM | ICD-10-PCS | |
|---|---|---|
| Reports | Diagnoses - why the patient was seen | Procedures - what was done |
| Used by | Every provider, every setting | Hospitals, for inpatient stays only |
| Format | 3 to 7 characters, letter first, decimal after the 3rd character | Exactly 7 characters, no decimal |
| Example | E11.9 Type 2 diabetes without complications | 0FT44ZZ Resection of gallbladder, percutaneous endoscopic approach |
| Maintained by | NCHS/CDC (with CMS) | CMS |
Where CPT and HCPCS fit
Outpatient and physician procedures are reported with CPT and HCPCS codes, not PCS. A single hospital stay can therefore carry ICD-10-CM diagnoses plus ICD-10-PCS procedures on the inpatient claim, while the surgeon's professional claim for the same operation uses CPT. If you only remember one rule: PCS never appears on a professional (physician) claim, and CPT never appears on an inpatient facility claim.
